At 9:05 pm on Friday, February 5, 2021 Bay District Station 9, and EMS stations 38 & 39 (LPVRS) were alerted to Three Notch Road @ MacArthur Blvd for a motor vehicle accident with injuries. In less than a minute Fire Chief 3 (J. Gould) was on scene reporting a two-vehicle accident, one car in the south bound lanes and one car in the northbound lanes. Moments later Safety 9 (J. Adams) arrived on scene and was directed to perform traffic blocking for the vehicle in the north bound lanes.

RE92 arrived on scene and began to contain the fluid leaks form one of the vehicles preventing it from getting into the storm drain. A389 & A397 arrived on scene and began to assess the occupants of the vehicles.

While units were operating on the scene a vehicle traveling north bound on Three Notch Road struck the Safety Officers Command Unit causing significant damage to both vehicles. The driver of the striking vehicle stated she was watching the emergency vehicles on the other side of the road and was not looking in front of her at the emergency vehicle blocking traffic for the scene of the accident.

All occupants of both incidents signed refusals for care form the EMS units on scene.
